HEALTH STATUS REPORT
All volunteers interested in working on disaster relief operations, both locally and nationally, must have an up-to-date Health Status Report on file with the Chapter. Health Status Reports must be updated every year and/or any time there is a change in your health status (i.e. surgery, diagnosed with a new condition).
The reason we ask these forms to be turned in annually is to protect you from becoming injured on a local or national disaster response. This is critical information which is reviewed only by our Staff Health Reviewers and is kept strictly confidential.
If your report needs to be updated, please download a fillable Health Status Report and submit back to the Chapter. Please contact Brenda Lawlor or Autumn Halliwell with any questions about this process. |
